San Francisco Community Clinic Consortium is a collaboration of 12 community health centers serving as San Francisco's primary care safety net system. For more than 40 years, SFCCC and its members have led efforts to enhance the well-being and diminish health inequalities among the city's varied low-income populations through direct care, specialized knowledge, and community advocacy.
Comprising 12 member clinics, the consortium caters to over 112,000 low-income, uninsured, and medically underserved individuals annually. Strategically located across San Francisco, these clinics offer a wide array of services including primary, preventive, ambulatory care, behavioral health, dental and vision care, and health education, fostering a comprehensive approach to healthcare delivery.
One of SFCCC's key initiatives, Street Outreach Services (SOS), delivers healthcare to homeless individuals with compassion and quality since 1988. Additionally, Veterinary Street Outreach Services (Vet SOS) tenders free veterinary care to homeless San Franciscans' companion animals while connecting their owners to vital healthcare resources, emphasizing the organization's holistic approach to community well-being.
